Indications/Uses
Prophylaxis & treatment of venous thrombosis & pulmonary embolism. MI & arterial embolism. Prevention of clotting in arterial & heart surgery; prevention of cerebral thrombosis. Anticoagulant in blood transfusions, extracorporeal circulation, dialysis & lab purposes.
Dosage/Direction for Use
IV 5,000-10,000 IU every 4 hr either by bolus inj or continuous infusion. SC 10,000 IU every 8 hr after initial 5,000 IU IV bolus inj. Low-dose heparin prophylaxis 5,000 IU 2-6 hr pre-op & every 8-12 hr post-op for 10-14 days. MI 5,000 IU SC every 12 hr beginning 12 hr following 1st sign of MI. Open heart surgery 120 IU/kg/hr for <2 hr op, 1.5 times more for longer op. Dose: 2,000 IU/450 mL. Peri-op prophylaxis Duration: 10-14 days. Established thrombosis As much as 6 wk.
Contraindications
Hypersensitivity. When suitable blood coagulation tests eg, whole blood clotting time, partial thromboplastin time cannot be performed at required intervals. Uncontrolled active bleeding state.
Special Precautions
Not recommended for IM inj. Discontinue use if haemorrhage occurs. History of allergy. Subacute bacterial endocarditis; arterial sclerosis; increased capillary permeability; during & immediately following spinal tap or anaesth, major surgery especially involving brain, spinal cord or eye; conditions associated w/ increased bleeding tendencies eg, haemophilia, some purpuras, thrombocytopenia; inaccessible GI ulcerative lesions & continuous stomach or small intestine tube drainage. Patients about to undergo surgery. Concomitant use w/ warfarin Na, other oral anticoagulants. Renal or hepatic impairment. Pregnancy. Not to be used in neonates. Avoid use in childn <2 yr.
Adverse Reactions
Transient alopecia, diarrhoea; thrombocytopenia, osteoporosis w/ spontaneous fractures; febrile or allergic reactions.
Drug Interactions
Induced bleeding w/ drugs interfering platelet aggregation eg, ASA, dextran, phenylbutazone, ibuprofen, indomethacin, dipyridamole, hydroxychloroquine. Partially counteracted anticoagulant action by digitalis, tetracycline, nicotine, antihistamines.
